Bethlem myopathy is predominantly an autosomal dominant myopathy, classified as a congenital form of limb-girdle muscular dystrophy. There are two types of Bethlem myopathy, based on which type of collagen is affected. Bethlem myopathy 1 (BTHLM1) is caused by a mutation in one of the three genes coding for type VI collagen. These include COL6A1, COL6A2, and COL6A3. It is typically autosomal dominant, though uncommonly can be autosomal recessive. Bethlem myopathy 2 (BTHLM2), formerly known as myopathic-type Ehlers–Danlos syndrome, is caused by a mutation on the COL12A1 gene coding for type XII collagen. It is autosomal dominant. In 2017, an international workshop proposed a redefined criteria and naming system for limb-girdle muscular dystrophies. Bethlem myopathy 1 (collagen VI) was included into the proposed list and renamed LGMDD5 for autosomal dominant mutations and LGMDR22 for recessive mutations. Bethlem myopathy 2 (collagen XII) was not addressed. === Differential diagnosis === Several disorders share some characteristics with EDS. For example, in cutis laxa, the skin is loose, hanging, and wrinkled. In EDS, the skin can be pulled away from the body, but is elastic and returns to normal when let go. In Marfan syndrome, the joints are very mobile, and similar cardiovascular complications occur. People with a "marfanoid" appearance are often tall and thin with long arms and legs and "spidery" fingers, while EDS phenotypes vary considerably. Certain subtypes of EDS may involve short stature, large eyes, and the appearance of a small mouth and chin, due to a small palate. The palate can have a high arch, causing dental crowding. Blood vessels can sometimes be easily seen through translucent skin, especially on the chest. The genetic connective tissue disorder Loeys–Dietz syndrome also has symptoms that overlap with EDS. In the past, Menkes disease, a copper metabolism disorder, was thought to be a form of EDS. People are commonly misdiagnosed with fibromyalgia, bleeding disorders, or other disorders that can mimic EDS symptoms. === Macrophages === One of the roles of macrophages is to phagocytize other expended phagocytes, bacteria and damaged tissue, and they also debride damaged tissue by releasing proteases. Macrophages function in regeneration and are essential for wound healing. They are stimulated by the low oxygen content of their surroundings to produce factors that induce and speed angiogenesis and they also stimulate cells that reepithelialize the wound, create granulation tissue, and lay down a new extracellular matrix. By secreting these factors, macrophages contribute to pushing the wound healing process into the next phase. They replace PMNs as the predominant cells in the wound by two days after injury. The spleen contains half the body's monocytes in reserve ready to be deployed to injured tissue. Attracted to the wound site by growth factors released by platelets and other cells, monocytes from the bloodstream enter the area through blood vessel walls. Numbers of monocytes in the wound peak one to one and a half days after the injury occurs. Once they are in the wound site, monocytes mature into macrophages. Macrophages also secrete a number of factors such as growth factors and other cytokines, especially during the third and fourth post-wounding days. These factors attract cells involved in the proliferation stage of healing to the area. In wound healing that result in incomplete repair, scar contraction occurs, bringing varying gradations of structural imperfections, deformities and problems with flexibility. Curium is one of the most radioactive isolable elements. Its two most common isotopes 242Cm and 244Cm are strong alpha emitters (energy 6 MeV); they have fairly short half-lives, 162.8 days and 18.1 years, and give as much as 120 W/g and 3 W/g of heat, respectively. As an α-emitter, 244Cm needs much less radiation shielding, but it has a high spontaneous fission rate, and thus a lot of neutron and gamma radiation. Compared to a competing thermoelectric generator isotope such as 238Pu, 244Cm emits 500 times more neutrons, and its higher gamma emission requires a shield that is 20 times thicker—2 inches (51 mm) of lead for a 1 kW source, compared to 0.1 inches (2.5 mm) for 238Pu. Therefore, this use of curium is currently considered impractical. A more promising use of 242Cm is for making 238Pu, a better radioisotope for thermoelectric generators such as in heart pacemakers. The alternate routes to 238Pu use the (n,γ) reaction of 237Np, or deuteron bombardment of uranium, though both reactions always produce 236Pu as an undesired by-product since the latter decays to 232U with strong gamma emission.
